What is entry and exit form of Vietnam Immigration Department

When you enter into Vietnam, you must fill out entry and exit form that is important in Vietnam Immigration for foreigners in Vietnam. You can get the entry and exit form by 2 following ways:

1. You can get entry and exit form at the Landing visa counter at the airport gate. You must fill out your private information in written by hand. It takes about 7-8 minute to fill out this form.

2. If you come to Vietnam by air and use visa on arrival. The visa agency that process visa will send you an email that contains an approval letter and a link to download entry and exit form. You can fill out your require information in this form and stick your photo and carry it to departure airport to get visa.

From number 1 to number 5 of entry and exit form is your private information on detail:

No1. Name and surname: you must fill out your name as in your passport. Note you should fill out the letters in block.

No2. Date of birth: you will fill out this section in dd/mm/yyyy format

No3. Sex: male or female

No4. Place of birth: you should fill out city and country name in this section.

No5. Nationality: include in nationality at birth and current nationality

No6. Passport details: You must fill out this section on your passport details:
Passport number: you should fill out this section exactly with your passport number (both number and alphabet letter)
Date of issue and expiry date
Issue by: in this section, you can check your passport detail to fulfill enter and exit form exactly.

No7. Profession, place of employment and telephone number. This information must exactly with the information in your visa application form

No8. Current address and telephone number: you must fill out your present address of your country.

No9. Children accompanying: you must fill out your children’s full name, date of birth and relation of you and children.

No10. Purpose of entry and exit: you must fill out your purpose to come Vietnam with your private reason such as: vacation, business, study or visit relative accompany with address and full name of your offices, organizations relations, address and full name of your relatives in case you visit relatives. You fill out this section to contact you while you are staying Vietnam.

No11. Propose duration to entry and exit Vietnam: the information must match the information in your visa.

No12. Entry and exit times: there are 2 type of entry time into Vietnam.
- Single entry: with this type of visa, you can enter into Vietnam one time in valid period
- Multiple: with this type of visa, you can enter and exit Vietnam more than one time in valid period.

No13. Port of arrival and departure: you can fill out this form only 3 airport in Vietnam. That are: Noi Bai, Ho Chi Minh and Da Nang airport

No14. Your confirmation: in this section, that is printed in advance with sentence: I swear the above declarations are true for which I shall take responsibility.
